“It is my great pleasure to be speak on behalf on the Wise Elephant Downtown Brampton Family Health Team.
It has been a hectic past few months getting our Family Health Team up and going. None of this would have been possible without our staff. We are very proud of our team. From our receptionists, to the lab staff, our interdisciplinary health professionals, our physician assistants, my fellow FHT medical colleagues including my wife Dr. Banerjee and our executive director, Jaipaul Massey Singh.
Of course, we wouldn’t be here if it weren’t for our community, and most specifically our patients – which number approx 9000 at this time.
Personally, having been in family practice for close to 15 years, Since the Family Health Team started, I see a huge difference firsthand in the quality of care we are able to provide to our patients. Here are just a few examples of what I mean.
Firstly, with our team approach and greater coordination amongst us, patients are getting seen on a timely basis, usually same day or next day 7 days of the week. Nothing makes a mother happier than having her child with a fever seen by her health team the same day rather than going to a walk in or wondering if she should sit in the Emergency Room. And that makes my day.
Secondly, we are fully computerized with all of our team members using the electronic health record. Our next step is to improve communication with those outside of our Family Health Team. Recently, we have been able to logon remotely to the Hospital portal. This is a huge benefit. For example, A patient of mine recently was discharged from the hospital. He was thrilled to see that with a few clicks I was able to access all his information, his medications and lab results. That makes my day.
Thirdly, our team of various skilled health professionals means that the right person gets seen by right health professional. I’d like you to picture this. It is Thursday evening, I have to get my kids to soccer and one of my last patients was a woman who just had a C section 2 days ago and she was having a lot of problems breastfeeding. Her appointment with the breastfeeding clinic wasn’t till tomorrow. I give some advice but as you must realize I just don’t have the experience. So desparately I called up our Nurse Practitioner Melissa who is very skilled in this area and she spoke to the patient over the phone. In fact, Melissa made a home visit to this patient that same evening. That was a relieved mother and it goes without saying it made my day and my kids day as I got home on time.
The right care, at the right time by the right person
We are constantly learning, and striving to improve the quality of care that we provide.
It is truly an exciting time to be a member of a Family Health Team in this province.
